SRMC-Sutter Roseville Med Center
Position Overview:
Coordinates and implements the transition of care (TOC) / Discharge (DC) plan for ambulatory patients. Prioritizes and coordinates the plan across the care of continuum through critical thinking, teamwork, and communication between care providers, patients, families and external vendors to ensure timely discharge. *Limited Term 26 Weeks
Job Description:
EDUCATION Equivalent experience will be accepted in lieu of the required degree or diploma.
- Associate's: AA in Business/Health related field or equivalent education/experience
PREFERRED EXPERIENCE AS TYPICALLY ACQUIRED IN:
- 1 year in a healthcare or hospital setting working directly with interdisciplinary team/patients/families
SKILLS AND KNOWLEDGE
- Oral and written communication skills.
- Interpersonal and time management skills
- Ability to work effectively in a fast pace environment with rapidly shifting priorities and competing demands.
- Ability to work independently with a minimum of direction. Ability to exercise discretion and prioritize tasks, seeking input as indicated.
- Intermediate PC skills and word processing skills required.

Pay Range is $30.01 to $42.01 / hour
