COMMUNITY HEALTH WORKER

**Location: Burnet Campus, 3440 Burnet Ave., Cincinnati OH 45229

Shift/Schedule: Day Shift/40 hours per week, Monday-Friday 8a-4:30p.  No evenings, weekends or holidays; no on-call.**

Required qualifications:

- Candidates must have **a high school diploma or equivalent

  • 2-5 years of work experience in a related job discipline
  • Have a reliable vehicle/valid driver's license**

Helpful information:

**- Current Community Health Worker (CHW) certification in the state of Ohio is preferred but not required

Job responsibilities:

**- Work collaboratively with care team of doctors, nurses, and social workers, among other specialties.

  • Coordinate care for patients and families, including medical and social needs.
  • Complete face to face visits, evaluate care plan progress; communicate results/coordinate next steps with the care manager.
  • Monitor ongoing needs of the patients/families to remove barriers, ensure efficiency and help improve access to care and resources.
  • Schedule appointments for members/families.
  • Complete required documentation linked to the care plan.
  • Provide updates to families regarding patient condition and progress; refer families to appropriate resources for information to answer questions and address needs.
  • Consult with care team members to address complex issues.
  • Supports population outreach to identify population(s) at risk and assist with closing population gaps.**
