What We’re Looking For ● License: RN (Registered Nurse) with active(state) license. ● Experience: At least 1 year of experience in home health or case management preferred. ● Skills: Strong critical thinking, communication, time management, and the ability to work independently. ● Tech-Savvy: Comfortable using EHR systems (experience with Kantime, Axxess, HCHB, or WellSky is a plus). ● Reliable Transportation: Valid driver’s license, auto insurance, and access to a dependable vehicle.
Why You'll Love Working With Us ● Supportive team culture where your input matters ● Manageable caseloads that respect your time and energy ● Flexible scheduling so you can balance life and work ● Opportunities for professional growth and continuing education ● A mission-driven environment focused on quality patient care
