Job Description:
- Promotes knowledge of oncology and its effects for patients, support partners, and care providers.
- Meets with patients and support partners after diagnosis to provide counseling and referral services specific to their needs.
- Collaborates with the multidisciplinary team to provide ongoing support for patients and oversee the plan of care.
- Educates and collaborates with the multidisciplinary team to plan and participate in quality initiatives and research projects.
- Reviews patient information and assess patients for emotional needs and risk factors.
- Serves as a customer service advocate for patients and support partners.
- Serves as the primary point of contact for providers and patients requiring the Oncology Licensed Social Worker (OLSW).
- Works with Navigators to provide Survivorship Care Plans to patients.
- Performs patient and family evaluations and histories, providing psychosocial support through individual or family counseling as needed.
- Acts as a resource to Infusion/Oncology Practices for patient needs and collaborates with patients to identify potential barriers to achieving oncology treatments and set health goals for quality of life.
- Other duties as assigned.
Knowledge, Skills, and Abilities:
- Able to communicate effectively in English, both verbally and in writing [Required]
- Additional languages [Preferred]
Education:
- Master's [Required]
Field of Study:
- in Licensed Social Work
Work Experience:
- 3+ of recent social work experience in oncology [Preferred]

$29.86 - $55.55
