  • Facilitate provider connections: Guide Care Coordinators in identifying appropriate fee-for-service providers based on member needs, preferences, location, availability, and service requirements.

  • Resolve escalated issues: Serve as the escalation point for complex member concerns, provider challenges, service access barriers, and coordination issues, providing guidance on appropriate non-clinical resources and follow-up.

  • Collaborate with stakeholders: Work with Provider Engagement Specialists, healthcare providers, internal teams, external agencies, and interdisciplinary partners to improve access to services and continuity of care.

  • Maintain documentation and compliance: Ensure member interactions, referrals, appointments, and follow-up activities are accurately and promptly recorded in electronic systems and comply with program, privacy, and HIPAA requirements.

  • Identify improvement opportunities: Analyze service gaps, workflow challenges, and recurring access barriers, then recommend and implement solutions that improve efficiency and member experience.

  • Manage staffing and coverage: Monitor schedules, staffing requirements, and workload distribution to maintain appropriate operational coverage.

  • Support quality improvement: Participate in operational meetings, leadership discussions, quality initiatives, and cross-functional workgroups. Communicate performance trends, concerns, and recommendations to leadership.

  • Promote culturally responsive services: Encourage respectful, inclusive interactions that reflect diverse member needs and help individuals and families navigate healthcare and community-based systems.

  • Ensure policy adherence: Reinforce corporate policies, ethical practices, confidentiality standards, and HIPAA Privacy and Security Rules across the team.

  • Perform additional duties and responsibilities as assigned.
